X-Ray Pre-Registration
You may pre-register using either of the following processes:
Using e-mail:
- Download the following forms
- Fill out the forms using your keyboard and save them
- Email the files to prereg@tshmail.com
Print and Fax:
- Download the following forms�
- Fill out the forms with your keyboard and print OR print the forms and fill out with a pen
- Fax the forms to 918-388-2736
MRI or CT Scan Pre-Registration
You may pre-register using either of the following processes:

- TSSH Pre-Registration Form
- Radiology Admission Agreement
- Medication Questionnaire
- Consent for Contrast (if your procedure involves contrast injection)
- MRI Questionnaire (if you are having an MRI)
- Fill out the forms using your keyboard and save them
- Email the files to prereg@tshmail.com
