Pre-register

To pre-register, please follow these steps. Need help?

Method #1: Pre-register digitally:

  1. Download the patient information form
  2. Open the file using Adobe Acrobat Reader
  3. Fill out the form using your keyboard and save it
    • Some versions of Adobe may not let you save the form.  You may need to update your version or use the fax method below.
  4. Email the file to prereg@tshmail.com

Method #2: Pre-register by fax:

  1. Download the patient information form
  2. Open the file using Adobe Acrobat Reader
  3. Use your keyboard to type in the fields, then print OR print the form and fill in the fields using a pen
  4. Fax a copy of the form to 918-388-2314

Accident?

If this visit concerns injuries sustained in an accident, please complete the accident disclosure form form as well.
